Description
FBA interview forms help you gather information from parents, teachers, and students when behavior observations do not tell the whole story. A student may respond differently across settings, and each person may notice patterns that others miss. These structured questionnaires give your team a consistent place to document those perspectives when completing a Functional Behavior Assessment or planning IEP behavior supports.
This resource includes parent, teacher, and student interview forms in printable and fillable PDF formats. Use them as written questionnaires or to guide conversations about strengths, concerns, behavior patterns, and strategies already attempted.
Gather Meaningful Input With FBA Interview Forms
Quick conversations and scattered notes can leave important questions unanswered. What happens before the behavior? When is the student most successful? What have adults already tried, and what does the student think would help?
These forms organize that information so your team can review it alongside observations and other assessment data. Gathering multiple perspectives helps you develop a more informed hypothesis about why behavior may be occurring and identify supports worth considering.
Parent Input Form for IEP Behavior Planning
The parent input form for IEP behavior planning gives families a structured way to share what they know about their child. It helps document strengths, concerns, and experiences outside school that may be relevant to understanding behavior.
The parent questionnaire addresses:
- Strengths, interests, preferred activities, and motivators
- Medical diagnoses, medications, and outside services
- What is going well at school and current concerns
- Problem behaviors observed at home
- Parent perspectives on why behaviors may occur
- Strategies used at home and whether they help
Use the FBA parent interview during a conversation with the family, or provide the questionnaire for written completion. Their responses can help your team recognize differences across settings and consider supports that build on the student’s strengths.
Teacher Interview and IEP Teacher Input Form
The teacher interview gathers classroom information that can be difficult to capture through a brief observation. Used as an IEP teacher input form for behavior planning, it helps your team review academic concerns, behavior patterns, and the history of support already provided.
Teachers share information about:
- Academic and social functioning compared with peers
- Academic skill needs and missing assignments
- Student strengths and interests
- Clear descriptions of problem behaviors
- Triggers, peer responses, and time-of-day patterns
- Whether academic difficulty may be contributing to behavior
- Reinforcement, reteaching, and correction strategies already attempted
The FBA teacher interview helps your team consider what is happening in the classroom before selecting an intervention. It also provides a record of previous strategies so you can build on what helped and examine why other approaches may not have been effective.
FBA Student Interview: Include the Student’s Perspective
Adults may agree that a behavior is a concern while understanding very little about how the student experiences the situation. The FBA student interview gives students an opportunity to describe when problems happen, what makes situations difficult, and what support they want from adults.
The student form addresses:
- Behaviors that lead to problems and how often they occur
- When and where those behaviors are most and least likely
- What makes behavior better or worse
- How teachers and peers respond
- Interests, friendships, and trusted adults
- Strategies that help when the student is upset
- Support the student would like to receive
Use the form as a written questionnaire or a guided interview, depending on the student’s communication and writing needs. Student input gives your team another perspective to consider and can help you involve the student in planning supports.

Use FBA Interview Interviews Alongside Other Assessment Information
Interview responses describe what people have noticed and how they understand the behavior. They can reveal useful patterns, but they do not establish the function of behavior on their own.
Review the responses alongside direct observations, ABC data, records, and other relevant assessment information. Differences between responses can also be useful: they may point to settings, expectations, relationships, or supports that need closer attention.
This resource provides interview forms, not a complete FBA or BIP template. Behavior observation tools, ABC data collection and graphing resources, and FBA planning templates are available separately.
