Description

This Autumn behavior point sheet gives teachers a consistent way to track daily behavior, provide specific feedback, and monitor progress toward a behavior goal. Three versions let you choose between scoring based on adult prompts or simple smiley-face feedback.

Student behavior can vary across activities and times of day. Recording feedback throughout the day helps you see when the student is successful, when additional support is needed, and what patterns deserve closer attention.

Choosing a theme connected to the student’s interests can help build initial buy-in. Clear expectations, direct instruction, and meaningful reinforcement remain the foundation of the system.

Three Behavior Point Sheet Versions

Choose the option that fits your student’s current needs:

  • Two or fewer prompts: The student colors the themed image when they demonstrate the target behavior with two or fewer adult prompts.
  • Three or fewer prompts: A more attainable starting point for students who currently need additional prompting.
  • Smiley-face feedback: A visual option for students who benefit from simpler ratings.

Choose achievable criteria, then review them as the student develops greater independence. If you change the scoring criteria, note that change when comparing results over time.

Create a Clear Daily Behavior Goal

A point sheet for behavior is most useful when the student understands what is being monitored and what success looks like. Define and teach the target behavior before using the sheet to score performance.

Use the fillable fields to enter student-specific information, such as the student’s name, target behavior, and schedule. For the prompt-based versions, establish an achievable daily percentage goal based on the student’s current performance.

Record feedback during each activity or time period. Review the completed sheet together to recognize success, discuss challenges, and identify what the student can try next.

How to Use the Point Sheet

  1. Choose the version that matches the student’s support needs.
  2. Type into the designated PDF fields or print the sheet and complete it by hand.
  3. Define and teach the target behavior.
  4. Explain the scoring criteria and how reinforcement will be earned.
  5. Record performance consistently throughout the day.
  6. Review results with the student and acknowledge specific successes.
  7. Use completed sheets to identify patterns and monitor progress.

For consistent data, adults should agree on the target behavior, what counts as a prompt, and how each section will be scored.

Support Self-Monitoring and Reflection

Students can help mark the sheet or complete their own copy to compare with the teacher’s ratings. This provides practice noticing their behavior, interpreting feedback, and identifying patterns across the day.

Teach self-monitoring through modeling and guided practice. Review ratings together until the student can evaluate their performance more accurately.

These conversations give students something specific to work with. Instead of hearing “do better,” they can discuss where they were successful, when they needed support, and which strategy might help next time.

Use Behavior Point Sheets for Progress Monitoring

Completed sheets can help you:

  • Identify patterns across activities and times of day
  • Review prompting needs
  • Compare daily results with an established goal
  • Share specific information with families and team members
  • Consider whether goals, instruction, or reinforcement need adjustment

For longer-term monitoring, daily points or percentages can be entered into a separate graphing tool. Graphing tool sold separately.

Share Meaningful Feedback With Families

The completed sheet gives families more information than a general “good day” or “difficult day” report. It shows performance across the day and provides a starting point for recognizing progress and discussing skills that still need support.

What’s Included

  • Three versions of the Autumn behavior point sheet
  • Scoring options based on two or fewer prompts, three or fewer prompts, or smiley-face feedback
  • Fillable PDF fields for student-specific information
  • Options to type into designated fields or print and complete by hand
  • Guidance explaining three important elements of effective reinforcement

What “Fillable PDF” Means

You can type into the designated form fields to add student-specific information. You can also print the sheets and complete them by hand.

The layout, graphics, directions, and scoring criteria are fixed. These elements cannot be moved, deleted, or changed. This is not an editable Word document or Google Sheets file.

Match Reinforcement to the Student’s Needs

The daily-goal approach is intended for students who can wait until the end of the day for the associated reinforcement. Students who are still developing that ability may need reinforcement more frequently.

Continue providing timely feedback throughout the day, even when the larger reward is earned at the end of the day.
