Check In Check Out Sheets (CICO): Daily Emotional Check-In & Reflection

SKU 13293851 Category

Description

When a student needs daily behavior support, it helps to have a consistent time to connect, set a goal, and talk through how the day went. These Check In Check Out Sheets (CICO) give those conversations structure while helping students practice emotional awareness, self-monitoring, and reflection.

With four printable forms to choose from, you can match the amount of visual support and writing to your student’s needs. Use them to build a daily routine in which students identify how they feel, choose a focus, recognize successes, and consider what to try next.

Support Connection, Goal Setting, and Reflection

These check in check out forms create regular opportunities for students to talk with a trusted adult. Morning prompts help students prepare for the day, while end-of-day reflections encourage them to notice progress and identify where they still need support. One format also includes a midday check-in.

The forms support students as they practice:

  • Identifying and communicating feelings
  • Choosing a specific daily behavior goal
  • Reflecting on their choices and effort
  • Recognizing strategies that helped
  • Planning what to work on next
  • Asking for and receiving adult guidance

Four Check In Check Out Sheets

Choose a check in check out sheet based on the student’s communication, writing, and reflection skills:

  • Emoji Behavior Tracker: Visual feelings choices, a daily focus, simple goal ratings, and space to draw or write about the best part of the day.
  • Basic Reflection Tracker: Morning, midday, and end-of-day check-ins with prompts for feelings, goal setting, progress, and adult encouragement.
  • Full-Day Self-Monitoring Tracker: Morning goal setting and end-of-day reflection to help students consider what went well and what they will work on tomorrow.
  • Focus & Reflection Tracker: Prompts that help students identify what supported their focus, consider what to do differently, and receive feedback from a teacher or mentor.

Teacher Guidance and Supporting Tools

Along with the four CICO forms, you’ll receive:

  • Teacher guide with setup suggestions, implementation tips, and ways to support different student needs
  • At-a-glance form comparison guide to help you select a starting format
  • Two-page Emoji Feelings Key to support emotion identification and communication
  • Single-student progress tracker to record morning emotions, chosen goals, end-of-day ratings, and reflection notes over time
  • Completed tracker example to show how information can be documented

Flexible Ways to Use These Forms

Use these sheets for a daily emotional check in, individual SEL support, or structured conversations with a teacher, counselor, or mentor. They can also support the reflection and relationship-building portions of a student’s check-in check-out routine alongside an existing behavior intervention plan.

The progress tracker helps you organize daily observations for team discussions, behavior planning, and IEP documentation when the information matches the student’s goals.
